How do I make an appointment?

What does the One Medical Seniors team provide?

Your One Medical Seniors team is a group of healthcare professionals who provide team-based care in order to address every part of your health and wellbeing. We don’t just want to treat your symptoms. We aim to provide a comprehensive approach to your care. Your provider and your Health Coach work together to get to know you and what’s important in your health so you can get the most of your care with us inside and outside of the practice.

What if I need to schedule an appointment after hours or over the weekend?

All One Medical Seniors practices are open Monday-Friday, and hours vary depending on the day. While we like to reserve a few dedicated hours throughout the week to coordinate with other doctors and specialists, as well as follow up with patients who require additional care, we are still available for our patients from 9 am to 5 pm during weekdays, should you need to give us a call.

Current patients can call their practice at any time. A One Medical Seniors provider is available 24/7 for current patients via phone. We also offer same and next day appointments for current patients with acute needs.

What do I need to bring to my first primary care visit?

  • All of the medications you take (this is preferred to a medication list), including inhalers, injections, supplements and over-the-counter medications

  • Any recent doctors’ notes or medical records you have

  • The name of your last primary care provider

  • Information on a recent hospitalization (if applicable)

  • Your insurance card
